Foundation Men’s Health 

 1. ABOUT FOUNDATION MEN’S HEALTH 

Foundation Men’s Health is an Australia-based telehealth medical service providing doctor-led assessment and treatment for men. All medical services are delivered by AHPRA-registered medical practitioners via secure telehealth platforms. 

Foundation Men’s Health is a medical service, not a wellness, coaching, or lifestyle platform. 

2. MEDICAL SERVICES OFFERED 

Services may include, but are not limited to: 

  • Testosterone Replacement Therapy (TRT)
  • Medical Weight Management
  • Sexual Health (Erectile Dysfunction)
  • Hair Loss Treatment


All services are provided only where clinically appropriate, as determined by a treating doctor.
 

3. NO GUARANTEE OF TREATMENT 

Completion of an online medical assessment, payment for pathology testing, or participation in ongoing care does not guarantee that treatment will be prescribed. 

Treatment decisions are made solely by AHPRA-registered doctors based on medical history, clinical assessment, diagnostic results, and applicable medical guidelines. 

4. MEDICAL ASSESSMENT & WORKFLOW 

The general clinical process includes: 

  1. Completion of an online medical assessment questionnaire
  2. Diagnostic pathology testing (2 tests required)
  3. Doctor review of results
  4. Medical consultation where clinically appropriate
  5. Ongoing care under a medical membership (if applicable)


Doctors may determine that treatment is not clinically indicated, in which case no prescription will be provided.
 

5. PATHOLOGY & DIAGNOSTIC TESTING 

Pathology testing may be required prior to or during treatment, is not included in membership fees, and is billed separately at the patient’s cost. 

Foundation Men’s Health does not control third-party pathology provider pricing. 

6. MEMBERSHIP, INITIAL PAYMENT & ONGOING CARE 

Where treatment is clinically indicated, patients may be offered ongoing care under a monthly medical membership. 

The initial payment covers: 

  • Initial medical assessment and review
  • Review of diagnostic pathology (where applicable)
  • A medical consultation where clinically appropriate
  • If ongoing care is commenced, this payment constitutes the first month of membership 


Ongoing membership may include:
 

  • Follow-up medical consultations
  • Review of pathology and clinical data
  • Ongoing monitoring and treatment adjustments
  • Clinical administration and governance


Membership does not guarantee prescription, continuation of treatment, or specific clinical outcomes. All clinical decisions remain at the discretion of the treating medical practitioner.
 

7. FEES, BILLING & PAYMENTS 

Membership fees are disclosed prior to commencement and are billed monthly in advance. 

Membership fees do not include medications or pathology. 

Medication costs are billed separately by dispensing pharmacies. Pathology costs are billed separately by third-party pathology providers. 

Membership fees are charged automatically to the payment method on file. Failure to maintain payment may result in suspension or termination of access to services. 

Foundation Men’s Health reserves the right to update fees with reasonable notice. 

8. CANCELLATION, REFUNDS & TERMINATION 

8.1 Patient Cancellation 

Patients may cancel their membership at any time by contacting Foundation Men’s Health in writing via email. 

Cancellation takes effect at the end of the current billing period, prevents future charges, and does not entitle the patient to a refund for the current billing period. 

8.2 No Refunds 

Membership fees are non-refundable, including where: 

  • Treatment is not prescribed
  • A patient chooses not to proceed
  • A patient does not book or attend consultations
  • A patient cancels part-way through a billing period

 

This reflects the availability of medical services, practitioner time, and clinical review. 

8.3 Clinical Discontinuation 

Foundation Men’s Health may discontinue treatment or terminate membership where ongoing care is no longer clinically appropriate, where monitoring or follow-up requirements are not met, where false or misleading information is provided, or where behaviour is abusive, unsafe, or inappropriate. 

Where membership is terminated for clinical reasons, no further charges will be applied. 

8.4 Breach of Terms 

Foundation Men’s Health may suspend or terminate access to services where these Terms & Conditions are breached. 

9. COMMUNICATIONS 

Foundation Men’s Health may communicate with patients via secure patient portals, email, SMS, or phone. 

Clinical communications may include confirmation of assessments, notification that treatment is not recommended, invitations to book consultations, and updates regarding ongoing care. 

10. PRIVACY & HEALTH INFORMATION 

Patient information is handled in accordance with the Privacy Act 1988 (Cth), Australian Privacy Principles, and applicable health records legislation. 

This policy applies to all users of the Foundation Men’s Health website and services, including patients, prospective patients, and website visitors.

  • Personal information we collect

We may collect personal information, including sensitive information and health information, that is reasonably necessary to provide medical and related services.

This may include personal information such as name, date of birth, contact details, identification details where required, and payment information processed via third-party providers.

Health information may include medical history, symptoms, clinical information, medications, treatment history, pathology and diagnostic results, and information required to assess and manage medical care.

We may also collect website and technical information such as IP address, browser type, device information, cookies, and analytics data.

  • How we collect information

Information is collected through online medical assessment questionnaires, telehealth consultations, direct communications, clinical reviews, interactions with pathology providers and pharmacies, and automated technologies such as cookies and analytics tools.

  • Why we collect and use information

Information is collected to provide medical and telehealth services, assess suitability for treatment, communicate with patients, prescribe and monitor treatment, maintain clinical records, process billing, conduct clinical governance activities, and comply with legal and regulatory obligations.

  • Disclosure of information

Information may be disclosed to treating practitioners, pathology providers, pharmacies, clinical software providers, payment processors, regulators, insurers, or other parties as required by law or with consent.

  • Data security and storage

Reasonable steps are taken to protect personal information from misuse, loss, unauthorised access, modification, or disclosure. Information is retained in accordance with clinical and legal requirements.

  • Access and correction

Individuals may request access to or correction of their personal information by contacting Foundation Men’s Health.

  • Complaints

Complaints may be raised directly with Foundation Men’s Health or escalated to the Office of the Australian Information Commissioner if unresolved.

  • Cookies and analytics

Cookies and analytics tools may be used to improve website functionality. Browser settings can be used to manage cookies.

  • Changes to this policy

This policy may be updated from time to time. The current version will always be available on the website.
